So lets assume that most of the rest of the world is not nuked. What happens with the US and USSR out of the picture?
We haven't quite worked out what happens in Europe. Though we think with Moscow gone the Red Army doesn't know what to do. Eastern Europe probbaly becomes a dysfunctional hell hole.
The Israelis and Arabs start fighting. Heck, the Israelis start tossing their own nukes around, no? If you wanted to make your point about not being messed with, what would you nuke?
India and Pakistan? Problem is Pakistan can't take on India and doesn't really have anything India wants. Mass panic in India.
Does China join in the nuclear exchange? Also wouldn't they invade Siberia? We expect the complete disintegration of the USSR.
We anticipate tons of help flooding in from South America. Argentinia takes the Falklands.
